Submit a National Provider Identifier (NPI)

What is your NPI, and why do we need it?

Your NPI is a unique 10-digit number issued by the Centers of Medicare & Medicaid Services. Once assigned to you, your NPI remains with you. Under HIPPA guidelines, your NPI needs to be used in administrative and billing transactions with us.
