Help filing a complaint, grievance or appeal

3 ways to file a complaint

You have the right to make your voice heard about your health care experience — whether it’s about us, your plan, a health service or provider. We’re here to make filing a complaint a little easier.

Get help from your state

Your state insurance office can help you if you don’t agree with a decision made by your health plan — or if you need to appeal a decision. Your state insurance office can also help if you’re living outside of the United States but have a grievance with a health plan that’s subject to U.S. health care reform laws.

Choose the state where your plan is issued, or where your employer is based.

The federal health care reform law includes rules about appeals, which many plans must follow. If your plan is covered by this law*, you can get help with your appeal by calling the Employee Benefits Security Administration at 1-866-444-3272.
