What is a C-SNP?

 

An Aetna C-SNP offers personal support to help you manage your chronic conditions. It’s for eligible members who have chronic conditions like:

 

  • Diabetes
  • Chronic heart failure
  • Certain cardiovascular disorders  

  • Diabetes
  • Chronic heart failure
  • Cardiac arrhythmias
  • Coronary artery disease
  • Peripheral vascular disease
  • Chronic venous thromboembolic disorder  

A C-SNP is a Medicare Advantage plan with Part D prescription drug coverage.

 

Plus, you get a personal care team, low medical and prescription copays and an Aetna Medicare Extra Benefits Card to help pay for certain everyday expenses.

The Aetna Medicare Advantage Chronic Condition Special Needs Plan, or C-SNP, may be right for you.

 

What is an Aetna® C-SNP?

 

These are Medicare Advantage plans that are specially designed for eligible people living with severe or disabling chronic conditions.

 

To be eligible for an Aetna C-SNP, you must be aged 65 or older and have at least one of the following conditions:

 

  • High blood sugar

  • Enlarged heart or fluid on the lungs 

  • Or any of the following cardiovascular conditions: 
     

    • Irregular heartbeat

    • Heart blockages

    • Poor circulation in the hands and arms or feet and legs

    • Blood clots in your legs or lungs

 

Every Aetna C-SNP plan member has a personal care team.

 

They focus on your unique health needs to help keep you well.

 

Your care team can help you find doctors and schedule doctor visits. They can also help you find safe housing, local transportation, meal services and more.

 

Your care team will consist of a:

  • Care coordinator

  • Nurse care manager

  • Social worker and more

Aetna C-SNP plans offer health coverage with added benefits. For instance, $0 copays on all visits to your primary care provider and certain specialists that treat your condition. This helps you get access to the care you need all at no cost to you.

 

$0 copay specialists include:

 

  • Cardiologists

  • Endocrinologists

  • Nephrologists

  • Pulmonologists

 

And you’ll have a $0 monthly plan premium.

 

You also get extra support to help pay for everyday expenses.

 

By qualifying for enrollment in the plan, Aetna C-SNP members get a monthly allowance on the Extra Benefits Card. This will help pay for everyday expenses including: 

 

  • Over-the-counter, or OTC, health and wellness products

  • Healthy foods

  • Certain utilities

  • Transportation 

  • Personal care products 

 

C-SNPs also include dental, vision and hearing aid coverage.

 

And members can get $0 or low-cost covered Part D prescription drugs at in-network pharmacies. This makes it easier to afford your needed medicines.

C-SNP FAQs

To qualify for an Aetna C-SNP, you‘ll need your provider to verify your condition. They will need to verify that you have at least one of these conditions:
 

  • Diabetes
  • Chronic heart failure
  • Cardiac arrhythmias
  • Coronary artery disease
  • Peripheral vascular disease
  • Chronic venous thromboembolic disorder


An Aetna C-SNP is only for Medicare members with at least one of these eligible conditions. We have to check and make sure that all new members have at least one of these conditions. If we find that a member has enrolled in a C-SNP but does not have one of these conditions, we must disenroll that member.

You can speak with a licensed Medicare agent. They can walk you through the steps to enroll.

 

 

As part of the enrollment application, you’ll need to complete a form that covers the details of your condition. This form is called the Prequalification Assessment Tool (PQAT).

 

 

Then, we’ll send a form to your provider. Your provider will verify that you have that eligible condition and that you qualify for a C-SNP plan. Your health care provider must respond by the end of your first month enrolled in the Aetna C-SNP plan.
